How safe is your hospital? See which Fresno-area hospitals got ‘A’ grades
Two Fresno County hospitals are among the safest in the nation, according to a nonprofit organization that studies patient safety and care.
The Leapfrog Group issues Hospital Safety Grade reports twice a year for nearly 3,000 general hospitals in the United States based on rates of preventable errors, accidents, injuries and infections, as well as the systems hospitals use to prevent those.
The group’s latest report, released May 1 and searchable as an online database, grades health care facilities across the nation on a scale of A to F — with A being the highest grade and F the lowest.

What are the safest hospitals in Fresno County?
Of the seven local hospitals The Leapfrog Group rated in the spring of 2025, two received A grades for hospital safety.
Kaiser Permanente Fresno Medical Center, 7300 North Fresno Ave. in Fresno, excelled at having effective practices to prevent medical errors in addition to good nursing and bedside care for patients, according to the Leapfrog Group report.
Adventist Health Selma, 1141 Rose Ave in Selma, scored high marks for having safe medication administration and efforts to prevent safety problems such as collapsed lungs and dangerous blood clots.
Both hospitals received A grades in the fall of 2024, according to previous reporting by The Fresno Bee.
Where are most unsafe hospitals in Fresno area?
Three Fresno-area hospitals received C grades in The Leapfrog Group’s latest report, including Clovis Community Medical Center, 2755 Herndon Ave. in Clovis.
According to the report, the Clovis hospital scored poorly due to unsanitary conditions resulting in staph infections and surgical site infection after colon surgery, as well as safety problems such as harmful events.
Clovis Community Medical Center dropped a grade compared to the fall of 2024, when The Leapfrog Group awarded it a B.
Community Regional Medical Center, 2823 Fresno St. in Fresno, also received a C grade after receiving low scores for poor bedside manner and responsiveness of hospital staff.
Meanwhile, Saint Agnes Medical Center, 1303 East Herndon Ave. in Fresno, scored low in categories including infections and safety problems.
Community Regional Medical Center and Saint Agnes Medical Center have received a mix of “C” and “D” scores since 2021, The Bee previously reported.
No hospitals in the Fresno area received D or failing grades in spring 2025, according to The Leapfrog Group’s latest report.
How did patient care at nearby hospitals score?
In its spring 2025 report, The Leapfrog Group included hospital safety grades for two hospitals in areas bordering Fresno County.
Adventist Health Hanford, 115 Mall Drive in Hanford, scored an A for effective leadership and practices such as hand washing and safely administering medication, according to the report. The hospital is in Kings County.
In Tulare County, Kaweah Health, 400 West Mineral King Ave. in Visalia, was awarded a B, with doctors getting accolades for ordering medications through a computer and working with staff to prevent errors, according to The Leapfrog Group.
Adventist Health Tulare, 869 North Cherry St. in Tulare, scored an A thanks to its effective leadership and doctors who are specially trained to care for intensive care unit patients.
The three hospitals are about a 45-minute drive from downtown Fresno.
Valley Children’s Hospital in Madera did not receive a score because children’s hospitals and specialty hospitals are not required to publicly report their safety records, The Bee previously reported.
The Madera County children’s hospital, which is less than a half-hour drive from downtown Fresno, was named a Top Children’s Hospital by The Leapfrog Group for the fifth time in 2024, according to a Dec. 17 news release.
How are hospitals graded by Leapfrog?
The Leapfrog Group grades hospitals on a scale of A to F based on “overall performance in keeping patients safe from preventable harm and medical errors.”
The group said it came up with its hospital safety grades by looking at up to 30 national performance measures from the Centers for Medicare & Medicaid Services, the Leapfrog Hospital Survey and other supplemental data sources.
Leapfrog then worked under the guidance of a panel of patient safety experts to select 22 “evidence-based measures” — divided into five key categories — and develop a scoring methodology.
Those categories are:
- Infections
- Problems with surgery
- Safety problems
- Practices to prevent errors
- Doctors, nurses and hospital staff
