What are best — and worst — nursing homes in Fresno area? See Medicare ratings

Choosing a nursing home in the Fresno area can be a difficult — and expensive — decision.

To help guide that search, the U.S. Centers for Medicare and Medicaid Services grades skilled nursing facilities using data including state health inspections and complaints.

The federal scorecard looks at factors ranging from residents’ well-being and building safety to staffing levels and COVID-19 vaccination rates.

The Medicare website also notes which nursing homes have been cited for abuse, although it does not give details on what they were cited for or how many citations they received.

The Centers for Medicare and Medicaid define abuse as “willful infliction of injury, unreasonable confinement, intimidation, or punishment with resulting physical harm, pain or mental anguish. It includes verbal abuse, sexual abuse, physical abuse and mental abuse.”

Nursing homes are rated on a five-star scale, with one star indicating the facility is “much below average” and five stars indicating it’s “much above average.”

There are a total of 47 nursing facilities within a 50-mile radius from downtown Fresno, according to the federal agency.

Where are highest rated nursing homes in Fresno area?

The Fresno area also has several highly rated nursing homes, according to the federal scorecard.

As of Thursday, April 3, these facilities had five-star ratings from the Centers for Medicare and Medicaid Services:

Two of the top-rated skilled nursing facilities previously received accolades for their quality care.

Veterans Home of California Fresno and Pacific Gardens Nursing and Rehabilitation Center were named the Best Nursing Homes in Fresno by U.S. News & World Report in 2025.

What are warning signs of elder abuse? How do I report it?

According to the California Department of Social Services, each California county has an adult protective services agency to help adults ages 60 and older, as well as dependent adults with disabilities ages 18 to 59, when “these adults are unable to meet their own needs or are victims of abuse, neglect or exploitation.”

The Fresno County Adult Protective Services Agency investigates “referrals of alleged abuse, neglect, self-neglect, financial abuse, exploitation and other forms of maltreatment,” its website said.

According to the Fresno-Madera Agency on Aging, warning signs that older adults or adults with disabilities might be experiencing abuse include:

  • Unexplained bruises or injuries
  • Changes in thinking, confused or disoriented
  • Angry, indifferent or aggressive
  • Belongings, papers or credit cards are missing
  • Hesitant to talk openly
  • Lacks necessities
  • History of substance abuse, criminal behavior, mental illness or family violence
  • Others have been added to their bank accounts or important documents
