State health leaders to visit Clovis hospital

California healthcare policy leaders will visit Clovis Community Medical Center from 10 a.m. to noon Friday as part of a statewide tour of Patient-Centered Medical Homes.

Assembly Health Committee Chair Dr. Richard Pan and Diana Dooley, California Health and Human Services secretary, will be among the visitors.

Patient-Centered Medical Homes care for patients by managing chronic conditions and maintaining preventive care. Teams of medical providers work closely with patients to coordinate every aspect of care in these homes.
